As the Clinical Director – Admissions, you are responsible for coordinating the referral to admission process for hospice services. You lead the admission and intake teams, and clinical liaisons, to ensure that eligible patients receive care in a timely manner, including obtaining clinical record information, payer verification, and scheduling informational visits/starts of care. You coordinate care with marketing, clinical operations, and regional leadership to help assure quality patient care and achieve growth. You are a key contributor to building trusting relationships with referral sources, patients and their families. This is a FT position based out of our Roanoke office that has oversight throughout our entire service area.
If you are a registered nurse with a Bachelor’s degree, have al least 5 years of professional nursing experience in hospice with 3 of those years being in Medical Surgical, Geriatrics, hospice, and at least one year in a clinical leadership, please apply today.

Good Samaritan Hospice is seeking a Nurse Practitioner to join our team.
RESPONSIBILITIES INCLUDE:
1. Face to Face visits:
a. Perform face-to-face visit with the patient, usually at the patient’s place of residence, no more than 30 calendar days prior to the start of the recertification period to gather clinical findings to determine continued eligibility for hospice care.
b. Document in designated section of the hospice medical record that he or she had a face-to-face encounter with the patient, including the date of that visit. The attestation of the nurse practitioner shall state that the clinical findings of that visit were provided to the certifying physician for use in determining whether the patient continues to have a life expectancy of 6 months or less, should the illness run its normal course. Documentation must include the above statement, accompanying signature of the nurse practitioner or physician assistant, and the date signed per Good Samaritan Hospice electronic documentation protocol.
c. Communicate unusual findings, including those that may indicate a need for a change in the patient’s hospice plan of care, to the medical director/attending physician and/or nurse case manager.
2. Collaborate and consult with the medical director, palliative director and interdisciplinary team regarding clinical care and treatment of hospice and palliative patients as well as evaluating appropriateness for services.
3. Establish a hospice plan of care and hospice admission appropriateness as needed when Hospice Medical Director is not available. This may include writing prescriptions as allowed by law. The NP may not certify the terminal illness (42 CFR 418.22(c)(1) & (2).
4. Appropriately prescribe medications within scope of practice and under supervision of Medical Director.
5. Actively participate in interdisciplinary team meetings and mini-teams on request.
6. Make visits to patient’s location based on patient need and plan of care.
7. Provide education/training to hospice staff and community health care providers regarding palliative and hospice care.
8. Provide patients that are faced with life limiting illness continuity of care between home, hospital and long term care facilities.

Good Samaritan Hospice has an exciting opportunity for a highly skilled and experienced Registered Nurse. We are currently searching for a Regional Clinical Director of nursing services who shares our passion for providing quality care during critical illness and end-of-life.

In this role, you will be able to provide meaningful leadership that will make a difference in the lives of our patients, families, team members, and community partners.
This position is responsible for the leadership and management of all nursing services in the Good Samaritan Hospice (GSH) service area. GSH is a 24-hour Hospice Program with satellite locations in the region. This role is instrumental in creating alignment with the organization’s mission, vision, core values, and standards of conduct. This position ensures implementation of and compliance with established nursing policies, procedures, and programs in accordance with local, state and federal guidelines. This position promotes collaboration, teamwork, and cohesiveness across all departments to ensure the continued pursuit of nursing excellence.
Job Requirements:
- Bachelor’s degree in nursing. Master’s degree preferred.
- Minimum of five (5) years hospice nursing experience, with at least two (2) years hospice nursing leadership experience, including direct supervisory responsibilities and financial resources oversight.
- A current, unencumbered RN license for the state of Virginia or compact license to be converted to a Virginia state license within 90 days of hire.
- Current CHPN (Certified Hospice and Palliative Nurse) certification, or must obtain (and maintain) CHPN certification within two (2) years of hire.
- Current BLS certification required.
- Extensive knowledge of hospice standards of practice, state and federal regulations, overall healthcare environment, and general hospice policies and procedures.
- Superior ability to build relationships, both internal and external to the organization.
- Strong ability to develop and execute a strategy for program excellence.
- Excellent communication, emotional intelligence, team building, critical thinking, decision making, and employee relations skills; with a passion for the work of hospice.
- Experience with EMR (electronic medical records) systems is a must.
- Ability to travel throughout the region.
