As a community Liaison, you will be responsible for assisting in referral development by cultivating, maintaining, and strengthening relationships with referral sources and educating the community on the hospice concept of care. You will also coordinate and participate in health fairs, senior expos, and other events, as well as develop and maintain relationships with community organizations.

We are currently seeking a FT Hospice Aides for our Roanoke based service area.

As a Hospice Aide with Good Samaritan Hospice, you will provide direct care to our patients, assisting them in activities of daily living under the direction and supervision of a Registered Nurse. We are seeking individuals who are Certified Nurse Assistant in the state of Virginia, with at least 1 year of experience, and who feel a professional calling to caring for patients with serious and complex illness at end of life.

We are currently seeking a FT RN Clinical Liaison for our Roanoke based service area.

As a RN Clinical Liaison, you will function as an educator, facilitator and ready resource for hospitalists, discharge planners and other referral sources within the hospital on hospice-related topics such as the referral process, eligibility criteria, symptom management, and reimbursement. You will also work with the admissions and community relations teams in census development and educational initiatives and strategies, considering data and trends within hospitals. This position also backs up the Admissions RN as needed.

We are seeking individuals who are licensed as a RN in the State of Virginia, and have at least three years’ experience in healthcare, highly preferred in hospice or end-of-life care and GIP.

This position will have a schedule of 4, 10 hour shifts, with flexible hours of 10AM - 8:30PM.

We are currently seeking a FT Quality Coordinator for our service area.

As a Quality Coordinator, you will provide coordination and leadership of professional nursing practice activities leading to exemplary outcomes in quality, safety and service.

Job Qualifications

• Education- Bachelor's degree in health care, or a similar field of study required.
• Experience - Minimum of four (4) years of experience in a healthcare setting within a quality or education based role. Experience in a hospice or home health setting preferred.
• Licensure and Certification - Registered Nurse (RN) required with current CPR/BLS certification. Must have a driver's license and maintain a clean driving record and automobile insurance. CHPN or CPHQ certification preferred.

You will work with the interdisciplinary Group to develop a plan of care, educate the patient and patient's family in the care of the patient, and make visits to the patients home as frequently as necessary.
